I was an egg donor twice. The first retrieval was in March of 2014 for Shady Grove in Rockville MD, and the second donation was in August 2014 for DEBUSA.
My donor number is 0713-78
On my paperwork I also see donor number 189339 listed so not sure if that second number is for DEBUSA. -
